“Eat less, exercise more.” This familiar advice is often heard in weight loss discussions. However, the process can be more complicated for those using GLP-1 medications to manage their weight.

Dr. Benjamin Duffy, an Obesity Medicine Specialist leading the ThedaCare Weight Wellness Solutions program, emphasizes the need for a personalized approach to treating obesity. “If you choose to use these powerful medications in your treatment plan, it’s best to select a program that includes the oversight of an obesity medicine specialist in order to minimize side effects and maximize your results,” he stated.

GLP-1 medications such as Ozempic, Wegovy (semaglutide), and Mounjaro (tirzepatide) were initially developed for managing blood sugar levels in type 2 diabetes patients. They also slow digestion and suppress appetite, aiding in obesity treatment by creating a caloric deficit necessary for weight loss. However, Dr. Duffy warns that this could lead to muscle mass loss.

Amanda Atkinson, an Exercise Physiologist with ThedaCare Bariatrics, recommends strength training alongside a high-protein diet to counteract muscle mass loss from GLP-1 usage. She highlights activities like weightlifting and rowing as essential components of any weight loss effort.

Dr. Duffy explains that muscle serves multiple health functions beyond aesthetics: “When most people think of muscle, they picture big biceps or chiseled abs,” he said. “But muscle is not just about appearance. It’s one of the most important organs in your body.”

Muscle aids calorie burning even at rest, helps manage blood sugar levels reducing diabetes risk, maintains strength and mobility with age, protects against weight regain after fat loss, and speeds recovery from illness or injury.

“I often refer to muscle as metabolic armor,” Dr. Duffy noted.

Strength training becomes crucial when using GLP-1s due to potential rapid weight loss causing metabolism slowdown through muscle loss. Dr. Duffy advises prioritizing strength workouts: “Building and maintaining strength is key because it can help prevent the loss of muscle mass that can come with the rapid weight loss associated with taking a GLP-1.”

Atkinson offers practical tips for starting strength training: begin slowly with dynamic stretching warm-ups; modify exercises if needed while ensuring proper form; choose enjoyable activities; seek trainer assistance if possible; maintain hydration and protein intake; allow rest days for recovery.

“In a world that often focuses on losing weight," Dr. Duffy concludes," I believe it’s just as important to focus on what you are building."
